Nursing Preceptorship Plan
ObjectivesPatient-Centered Care: To focus on the patient’s particular health care needs, and act as an advocate for each patient. To strive to provide a holistic level of care that meets all the patients’ needs. To work with each patient to create a care plan that works for them, and focuses on empowering them to care for themselves whenever possible. Communication: To be comfortable communicating with patients and other health care providers, and to build therapeutic relationships with both. To use the verbal and nonverbal communication skills and assessments learned in school. Providing thoughtful and appropriate language to each patient and family in order to communicate effectively. Systems Based Practice: To recognize and appreciate my own impact on the care provided and surrounding environment as well as the impact of the other aspects of a health care organization. To value all facets of the working system and to effectively utilize all available resources to provide the best level of care possible.Quality Improvement: To actively participate in quality improvement methods and to seek information on current initiatives already in place. Constantly evaluate care processes for areas that could benefit from a change. Safety: To effectively minimize the risk of harm to my patients, self, and co-workers, by demonstrating safe practices and using communication when necessary to educate others on safe practices. To learn from nurses and others the protective measures in place in the facility and to appreciate why safety measures are in place. Observe and communicate any concerns related to hazards in the environment to my preceptor or other employee. Leadership: To recognize and seek out a quality leader for a mentor, and to understand that one’s attitudes, beliefs, work ethics, and experiences all affect one’s leadership style. To strive towards gaining leadership skills that can be used to lead others including patients. Recognize that a good leader is one who anticipates consequences, plans ahead, collaborates with others, and communicated effectively. Evidence Based Practice: To provide care that is based on the best available evidence moderated by patient circumstances and preferences, that is applied to improve the quality of clinical judgments.  To be able to critique information in order to integrate evidence based practices into patient care when applicable. To share new knowledge with others, and to be opened to continuing my own learning. Professionalism: To demonstrate my professionalism by showing equality and respect toward patients, colleagues, visitors, and staff.  To be accountable for providing a standard of care that upholds my ethical, sociocultural, legal, and humanistic obligation to patients and to my workplace. To incorporate the ANA’s Code of Ethics into my professional practice and personal goals. Informatics & Technology: To better understand the application and informatics and technology in the nursing field and to use it when appropriate.  To use technology safety to improve the quality of patient care. Teamwork & Collaboration: To work effectively in an interdisciplinary team, by listening, communicating, and contributing to the group. To ask for assistance when needed, and to provide assistance to others. Recognize my own role in the team, and how what I do impacts the whole. Geriatrics: To better understanding the risks associated with patient care of geriatric patients. To recognize differences in medication management due to metabolic changes. To identify home care issues and risks. To include the patient in a plan of care that is appropriate for them. To identify patient concerns as they relate to aging.
