Case management focuses on the strengths and concerns a client brings. Case management serves purposes are a method for determining an individualized service plan for each client and monitoring that plan to be sure it is effective, also it is a process used to ensure that the money being spent for the client’s services is being spent wisely and in the most efficient manner. To make sure that the purpose will be done, case management responsibilities have been separate into four types of service: assessment, planning, linking and monitoring. These services are the method that can be used to identify and assess the needs of the client, and develop a plan for effective services or treatment. At the same time, to identify and link the client to appropriate services and resources, it is important to prevent duplication. The principles and goals of case management. First, it is the integration of services, we need to realize that we are working with the whole person, and accept the many human points of view that considered in service delivery social, psychological, medical, financial, educational and vocational. It is very important to provide integrated the needed services for the multi-problem clients, these integrated services can bring together a variety of services to help a client. With the service integration, the benefit is reduced duplication and fragmentation of services, also facilitate the effective priority setting for services provided.

Second, it is the continuity of care, we need to make sure that the services for the client will not be interference, since we meet the client at the first time. Also, it is important that the therapeutic intervention along with support in the environment, for an example: client’s family and significant peoples. To make sure we provide the comprehensiveness care.

Third, it is a client empowerment, we need to respect clients is an individual and ensuring the client’s full participation during the service. These are the main principles and goals of case management, when we implement case management, we need to pay attention to those points.
In fulfilling the role of broker in case management, my strengths are good at finding out the needs of other people, I am also good at looking for different information and services. These strengths allow me more easier to help the client choose the most appropriate service. For my weaknesses are sometimes I might be more subjective, it may produce some bad effect when negotiating the terms of service delivery with the client, so I need to pay more attention to objectively negotiate.

At the same time, I have a good judgment that makes me could fulfill the role of consultant in case management. Because a consultant may have the expertise to identify the problem, study it and make recommendations, I can have a critical thought that based on facts, figures, problems. And make the best recommendations for the client.

The most important ability for me is organizational capacity, this strength lets me fulfill the role of planner. I
