Mr. ME loves to chat and jokes with staff and residents. He is very sociable person. He loves to go to the town, visits the charity shop, feed ducks and going to the pub. He always expresses his needs and wants. As a support worker, I always try to support and make arrangement for him. I accompany him and walk beside him as he needs to hold my arms to keep him from falling due to his poor mobility.

He is risk of trips and falls when walking in far distance, needs to support him when crossing the streets, he is anxious of people and dogs staring at him for too long. During handover and by reading the care plan, I can access information what support was given and needs to be given to him in order he can access services and facilities. Support workers and other staff share the information and review and agreed other options in case the individual cannot go out during the day or access services or facilities as well as identifying any risks and how to manage the risks. Individuals are informed and consulted if they agree to other options.

To provide active support for the service users, I try to prompt them to use the toilet and constantly encourage them to avoid themselves being wet. I try to remind individuals the importance of personal hygiene. I arrange with individuals proper time to use the facilities and make sure I supported them properly.

I have always aware that it is important that services and facilities such as toilets are used often to avoid incontinent issues. It is very helpful to individuals to make sure the facilities are clean, free from risk and hazard. Keeping the service users clean, free from odour and with good personal hygiene improve their well being and quality of life.

Individuals are often asked what they prefer and give feedback on the services and facilities used such as on meals, shopping, holidays, activities, education and leisure.

I make sure each individual are offered or asked if they want to participate in the activities such as going to pubs, art activities, walking to town and social clubs. I make sure each individual express what they want and give feedback on their experience. I asked them if they enjoyed, they like doing it again next time or offered them other services if they dont like it.

During handover, staff discuss the outcome of any use of facilities and services. Identified any changes when facilities was used, what facilities are effective and useful. Discuss the reason for the efficiency or less efficiency. All staff discuss, contribute and share information. It must be accurate, relevant, easy to understand and properly recorded and report the feedback, outcomes of the evaluation based within the confidentiality agreement and according to legal and organisational requirements.
