Discussing Treatment Plan
Larry McCune, at the age of 34, came in with a number of serious issues that are deeply affecting his daily life. He was involved in a car accident that took his 4 year old daughter’s life. Since then he has been dealing with severe headaches and nightmares. He has been recently divorced and his son, who is 9, now lives with his wife. Larry has visitation with his son that does not occur on a regular basis. He is having trouble with coworkers and dealing with a phobia about riding in any type of vehicle. In this treatment plan Mr. McCune’s strengths and weaknesses must be discovered and used to his advantage. Mr. McCune was very depressed and anxious during the interview. He stated that the main reason he was in the office was to help himself feel better and be able to work consistently.

Description of Treatment Plan
The first approach for Mr. McCune is to address his severe depression, guilt, and anxiety. This should be approached by referring him to group therapy sessions and a counselor. Mr. McCune may need to be medicated for his high levels of anxiety and depression. Another very important aspect that must be looked into his headaches. We have agreed that he should be evaluated for any residual neurological issues that could have resulted from the accident.

Family
Another aspect is Mr. McCune’s family that he is currently distant from. He sees his 9 year old son but not consistently. It is very important that he begins to rebuild a relationship with his surviving child. This can be done through family therapy and time. His personal state of mind must be worked on before this can occur. His relationship with his ex-wife seems to be on a level where they can communicate without hostility.

Work
His history with work after the accident has been very difficult due to his fear of transportation and irritability with coworkers. This is an aspect that must be approached in a gentle manner. The counseling will help him deal with his fear of transportation as well as his irritability with coworkers. Larry must learn to deal with his feelings and learn to remain calm with others.
